BRING!

To ensure a smooth and effective consultation, please bring the following:

  • Current X-rays: You can ask your dentist to send them directly to:

         periodex@hotmail.com

  • A list of medications you are currently taking.

  • Insurance information for billing purposes.

  • Referral notes or forms from your dentist, if applicable.
